Registration: April 1 through May 23, 2008, after May 23 there is a $10 LATE FEE per registrant

You can register for camps that overlap, work with the coaches regarding this issue.

Completed form (including parent signature), check (made out to RMHS) can be mailed to: Rolling Meadows High School, ATTN: SUMMER SPORTS CAMP

REFUND POLICY (Registration Fee is excluded):

• No refund for absence due to vacations, after 1/2 of the days of camp or sessions canceled due to inclement weather.
• Full refund of all fees if camp is canceled or doctors note are prorated by day, based upon date of refund.

INSURANCE

Students are responsible for their own insurance coverage. If a participating student is injured, the incident should be reported as if the student is a visitor on school premises. All injuries must be reported to the head coach/sponsor and trainer (if on duty). The head coach/sponsor must complete an accident report.
